CSF MEMBERSHIP APPLICATION FOR PALMA HIGH SCHOOL

A. To Qualify for this semester:

1.          You must earn a minimum of 10 points from last semesterÕs grades.

a.          The first four points must be from LIST I (unless you are a senior applying for membership in February or June.)

b.         The first 7 points (including the four points described in a) must be from lists I and II.

c.          The remaining points may come from any list (I, II, or III).

2.         You must use no more than 5 courses to qualify.

3.         No CSF points are given for P. E., courses taken in lieu of P. E., subjects repeated to improve a grade, and courses taken on a pass/fail basis.

4.         CSF points are granted as follows:

a.          a grade of A = 3 points

b.         a grade of B = 1 point

c.          One additional point shall be granted for a grade of A or B in an Honors or AP course, up to a maximum of two such points per semester

d.         A grade of C = 0 points

e.          A grade of D or F in any course disqualifies you from membership at this time.

5.         You must earn a minimum of 10 points from last semesterÕs grades.

                           IF NOT, THEN DO NOT simply turn in an application!.....See Mr. Rawson in person at the Main Office and make sure he writes down your                  concern/issue.  Also ask Mr. Rawson for a hand-written note to confirm what took place.

Make sure your name is printed along with all of the other names of accepted members in the next school newsletter that is sent in the mail. Failure to alert Mr. Rawson of an error on his/your part regarding your membership status (AS PRINTED IN THE NEWSLETTER) within 1 week of its mailing will lead to your membership being denied for the semester. Ask Mr. Rawson for a written record of any such problem.
